After software update 1.4.0 I was excited to create a group and send a message. I added my brother and then my partner (they’ve never been in a group before on this phone) and then I typed a message and hit send. Unfortunately, it appears that instead of creating a group chat and sending a message in the group chat, it is sending individual messages from me to each person in the group. Furthermore, it now appears all of my group chats are functioning in this manner and every one of my group chats are now broken in this manner.

It’s a setting you can change. Open your Messages app, go to the settings in the upper right, and click on your SIM card. It should give you the option to switch back to group MMS, instead of sending separate SMS messages to everyone. I have no idea why they chose that as the default – it threw me for a minute too.

You’re right, I didn’t switch over from the default “Separate SMS chats” to the “single MMS group chat”. Thanks for your help!
